Common use of Amendment of PHI Clause in Contracts

Amendment of PHI. i. Business Associate shall within ten days of receiving a written request from Covered Entity make any amendment to PHI in a Designated Record Set as directed by or agreed to by Covered Entity pursuant to 45 C.F.R. 164.526, or take other measures as necessary to satisfy Covered Entity’s obligations under 45 C.F.R. 164.526. ii. Business Associate shall promptly forward to Covered Entity any request for amendment of PHI that Business Associate receives directly from an Individual.

Amendment of PHI. i. Business Associate shall promptly forward to Covered Entity any request for amendment of PHI that Business Associate receives directly from an Individual.

Within ten (10) business days of request from Covered Entity, Business Associate shall within ten days of receiving a written request from Covered Entity make any amendment amendment(s) to PHI in a Designated Record Set as directed by or agreed to by Covered Entity pursuant to 45 C.F.R. 164.526Set, or take other measures as necessary to satisfy Covered Entity’s obligations under 45 C.F.R. CFR § 164.526. ii. Business Associate shall promptly forward will not respond directly to an Individual’s request for an amendment of his PHI held in the Business Associate’s Designated Record Set. Business Associate will direct the Individual to the Covered Entity any request for amendment of PHI so that Business Associate receives directly from an Covered Entity can coordinate and prepare a timely response to the Individual.
