Amendment of Protected Health Information. Participant #2 agrees to make any amendment(s) to Protected Health Information it holds in a Designated Record Set, as requested by an Individual or directed by Participant #1 pursuant to 45 CFR § 164.526. In the event the request for an amendment is delivered by an individual directly to Participant #2, it will promptly forward the request to Participant #1 and upon approval Participant #1, amend the Protected Health Information and incorporate the amendment into its records.
