An unexpected emergency. The employee shall, if required by the company, produce a medical certificate or statutory declaration made by the employee for the illness or injury of the person concerned, or in the case of an unexpected emergency, a statutory declaration. The term “immediate family” includes: • A spouse (including a former spouse, a de facto spouse or former de facto spouse) of the employee. A de facto spouse, of an employee, means a person of the opposite sex to the employee who lives with the employee as a husband or wife or that person on a genuine domestic basis although not legally married to the employee; and • A child or an adult child (including an adopted child, or a step child), a parent, a grandparent, a grandchild or a sibling of the employee or of the employee's spouse. The employee shall give the employer notice of the requirement and reasons for taking carer's leave as soon as reasonably practicable, preferably prior to the intention to take leave.
