APPLICATION BY MEMBER. Application for electric service (“Service Agreement”) is available in paper format from DSO and in electronic format on DSO’s website. The completed application becomes a service agreement or contract when accepted by DSO, or upon establishment of service. DSO may require a separate Service Agreement for each service at the same location or at each separate location. Applications that have not been completed (i.e., deposit paid, e-sign document returned, etc.) within 30 days of the application date will be voided.
