Application Procedures. Information regarding application procedures and conditions for all leaves is available from the District’s Human Resource Department and online at ▇▇▇▇://▇▇.▇▇▇▇.▇▇▇/Forms.html. 11.2.1 The principal/program supervisor shall be informed in writing by the employee and the appropriate leave application form completed and sent to the Director of Human Resources at least two (2) calendar months, if possible, before the commencement of the intended leave. 11.2.2 If the reasons for a parental leave include pregnancy, the employee shall also provide, at the time of leave application, a signed statement by her physician indicating the expected date of delivery and that the employee is fully capable of meeting the requirements of her position until the date of the leave. In the case of adoption, the employee shall provide to the Human Resource Department, at the time of application for adoption, notice that application has been made, and the best available information as to probable date of arrival of child.
