Common use of Arbitration Form Clause in Contracts

Arbitration Form. Each notice of arbitration shall be submitted in writing on the form shown in Appendix ―D‖. All pertinent information submitted with the Appendix ―C‖ form, as well as the Grievance Hearing decision, shall be included as an attachment to the Appendix ―D‖ form.
