Authority to Bind Contractor. Notwithstanding anything in this Contract to the contrary, the signatory for the Contractor represents that he/she has been duly authorized to execute contracts on behalf of the Contractor and has obtained all necessary or applicable approvals from the home office of the Contractor, if applicable, to make this Contract fully binding upon the Contractor when his/her signature is affixed, and this Contract is not subject to further acceptance by Contractor when accepted by the State of Indiana.

Authority to Bind Contractor. Upon receipt of a fully executed copy of this Agreement, the Contractor shall furnish to the City a written list of the persons authorized to execute, on behalf of the Contractor, agreements, contracts, modifications to contracts, or other documents as they may be required by the City. In the event authority is delegated to a position (e.g., President, Vice- President, Treasurer) rather than to an individual, the list of positions and individuals filling those positions so authorized shall be furnished. If any changes should occur to this list during the term of this Agreement, a revised copy shall be promptly furnished to the City.

Development of an Implementation team a. Current Provider policies and procedures, as well as intake documents and case plans to assist with the development and implementation of new screening/assessment approaches- to include use of the ADBTT. b. The implementation Committee will develop locally based intervention and treatment response matrix for matching of typology to services available in communities the ADBTT is administered. c. Implementation Team will develop/modify existing safety contracts for youth and family to help ensure the use of healthy coping skills d. Team and Consultant will review Quality Assurance and data tracking plans. B. Training Schedule Screening, assessment and process Training - Two days of in- person training on screening, assessment, safety planning, case plan development, services to match risk/need to supervision and treatment interventions, family involvement, continuity of care through completion of screening/assessment utilizing the ADBTT.
