Common use of Authorization Form Clause in Contracts

Authorization Form. The form of such authorization shall be as follows: ASSIGNMENT AND DEDUCTION AUTHORIZATION As of the date shown above, I hereby assign from my wages, and you are hereby authorized and directed to deduct regular Union dues, as certified by the Financial Secretary of Local Union 2088. The sums deducted shall be remitted by you to the Financial Secretary of Local 2088, of the International Brotherhood of Electrical Workers, AFL-CIO, in accordance with the provisions of the Agreement between you and said Union now in effect. This authorization shall be in effect for the duration of this Agreement or during any extension thereof, unless terminated by me in writing, addressed to the Utilities Commission with a certified copy to the Union.
