Authorization of Leave. The authorization of a leave of absence without pay is a matter of administrative discretion. The Employer or its designated representative shall decide in each individual case if a leave of absence is to be granted within the limitations of the appropriate rules of the Employer. A leave of absence shall be requested and authorized on a form designated by the Employer. All requests for leaves of absence without pay must be applied for thirty (30) calendar days prior to the commencement of the desired leave if possible.

Authorization of Leave. An employee will make every effort to notify the manager in advance of scheduled medical and dental appointments. When an employee must be absent because of illness or injury, a call should be made to the manager as early as possible into the shift, with an indication of the amount of sick leave that may be required.
