Benefits to Which Subscribers Are Entitled. 1. Our liability is limited to the Benefits specified in this Benefit Plan. 2. Benefits for Covered Services specified in this Benefit Plan will be provided only for services and supplies rendered on and after the Subscriber’s Effective Date by a Provider specified in this Benefit Plan and regularly included in such Provider’s charges.
