Common use of BILLING NUMBER Clause in Contracts

BILLING NUMBER. The number used for billing of all hospice care is the hospice program’s Employer Identification Number (EIN). This number must be used until the provider is officially notified by DHA of a change. The hospice program’s billing number is shown on the face sheet of this agreement.
