The 'Birth certificates' clause requires parties to provide official documentation verifying the birth of an individual, typically for identification or legal purposes. In practice, this clause may obligate one party to submit a certified copy of a birth certificate to confirm age, citizenship, or familial relationships, such as when enrolling a child in school or establishing eligibility for certain benefits. Its core function is to ensure the authenticity of personal information and prevent fraud or misrepresentation in legal or administrative processes.

2. Application for domestic partner coverage must include all of the plans in which the employee/retiree is presently enrolled; that is, the employee/retiree may not choose to enroll the domestic partner under only the dental but not the medical and vision, etc.

Birth certificates. All Players must submit a copy of an official Birth Certificate at the time of registration, per the requirements of US Club Soccer, of which ▇▇ ▇▇▇▇▇▇▇ is a member organization.
Birth certificates. All Players must submit a certified copy of a State-Issued Birth Certificate at the time of registration (via electronic upload). Hospital Certificates will not be accepted. No player can or will be registered without the State- Issued Birth Certificate.

Making good choices in how your student is dressed is important to help maintain a quality learning atmosphere for all students throughout the school day. Therefore, the guidelines for appropriate dress will be enforced for ALL STUDENTS at Prairie View and include:
Birth certificates. All new players are required to supply a copy of the player’s birth certificate to the ice office before tryouts/evaluations.
Birth certificates. We will need a copy of each dancer’s birth certificate. If you were a dancer last year, we should still have your copy (but if not, we will ask for another). Any new dancer, please give a copy of your birth certificate with the other information returned with this packet.
