CAMPAIGN CONTRIBUTION DISCLOSURE FORM. Offeror must complete, sign, and return the Campaign Contribution Disclosure Form Form, below, as a part of their proposal. This requirement applies regardless whether a covered contribution was made or not made for the positions of Governor and Lieutenant Governor or other identified official. Failure to complete and return the signed unaltered form will result in disqualification.
