Common use of Case Records Clause in Contracts

Case Records. CONTRACTOR shall maintain current case records for each PROGRAM PARTICIPANT and PROGRAM APPLICANT referred. ADMINISTRATOR will provide training, as mutually determined by CONTRACTOR and ADMINISTRATOR, regarding use and maintenance of case records. CONTRACTOR shall maintain case records, which shall be in a format approved by ADMINISTRATOR. Information in all case records shall be treated as confidential and released only to ADMINISTRATOR as required, or to others upon approval by ADMINISTRATOR. Items in the case records shall include, but are not limited to the following for each PROGRAM PARTICIPANT: Advance Directive/Power of Attorney. Health Insurance Portability and Accountability Act (HIPAA) Releases as needed. HEP if provided by PROGRAM PARTICIPANT. Program application. Housing agreement. TILP and amendments. Intake Summary. Needs and Services Plan and updates. Interviews with PROGRAM PARTICIPANT. Special Incident Reports. Social history report. Monthly evaluations. Progress notes, school performance, completed vocational assessment and employment attainment and progress. PROGRAM PARTICIPANT’s financial information, including revenues and disbursements for clothing and material provided by COUNTY and signed for by PROGRAM PARTICIPANT; wages and other incomes; allowances and incentives received by and signed for by PROGRAM PARTICIPANT. Documentation of all services provided, including contacts with and on behalf of PROGRAM PARTICIPANT and general observations. Documentation of community organizations working with PROGRAM PARTICIPANT. Child care arrangements/documentation. Documentation/justification for Supportive Services. Documentation of hours of participation. Documentation regarding any cooperation issues and cause determinations. Family connections. Employment information and employment retention tracking. Documentation of changes in earnings. Standard release forms as needed for collateral contacts. Documentation of language needs and how they were addressed, as applicable. Copies of rights and responsibilities and other forms and documents required CDSS, CCLD, ADMINISTRATOR, or in program procedures. Medical verifications, as applicable. Medical/dental records of visits/treatment. Quarterly Performance report. Termination Summary.
