Certification Required. F.4.5.1 An employee must, when applying for maternity leave or paternity leave, provide the employer with a medical certificate which : 1. names the employee or the employee’s spouse as appropriate; 2. states that the employee or the employee’s spouse is pregnant; and 3. states :- - the expected date of birth - the expected date of termination of pregnancy; or - the date on which the birth took place; whichever is appropriate. F.4.5.2 At the request of the employer, an employee must, in respect of the conferral of parental leave, produce to the employer within a reasonable time a statutory declaration which states: 1. the particulars of any period of parental leave sought or taken by the employees 2. that the employee is seeking the leave to become the primary care-giver of a child; 3. In the case of adoption leave, a statement from a Government authority giving details of the date, or presumed date, of adoption; and 4. that for the period of the leave the employee will not engage in any conduct inconsistent with the employee’s contract of employment.
