(4) 12 d. A dependent ceasing to satisfy eligibility requirements for coverage due to attainment of 13 age, student status, marital status, or other similar circumstances or otherwise no 14 longer meets the eligibility requirements under Section B(3).2C.
(5) 16 e. A change in the place of residence of the ASF Memberemployee, retiree or their spouse, or dependent that is not in the health plan service administrator’s area.or
(6) 19 f. Significant cost or coverage changes (including coverage curtailment and the addition 20 of a benefit package).
(7) 22 g. Family and Medical Leave Act (FMLA) leave.
(8) . 24 h. Judgments, decrees or orders.
(9) 26 i. A change in coverage of a spouse, spouse or dependent under another Employer's ’s plan.
(10) 28 j. Open enrollment under the plan of another Employer.
(11) 2 k. Health Insurance Portability and Accountability Act (HIPAA) special enrollment rights for 3 new dependents and in the case of loss of other Insurance insurance coverage.
(12) 5 l. A COBRA-qualifying event.
(13) 7 m. Loss of coverage under the group health plan of a governmental or educational institution (a State's children's health insurance program, medical care program of an 9 Indian tribal government, State health benefits risk pool, or foreign government group 10 health plan).

(15) Any other situations in which the group health or dental plan is required by the applicable federal or state law to allow a change in coverage.

Changes Due to a Life Event.
