Biweekly incentive premiums will be available to the primary member upon completion of all elements of the annual biometric testing, regardless of the biometric results. Incentive earnings will default as a Premium incentive unless primary member on plan indicates that incentive earnings should be placed into the Health Savings Account or Health Reimbursement Account. City provided basic Life insurance will increase to $25,000 for all eligible regular full-time IUPA bargaining unit members as of benefit year starting 1/1/2020 (or after if the CBA is not ratified by that date). This will be considered the basic life insurance policy that is offered and paid for by the City of Sarasota. B. The City will provide Long Term Disability (LTD) coverage for eligible regular full-time IUPA bargaining unit members as of benefit year starting 1/1/2020 (or after if the CBA is not ratified by that date). The City will incur the cost of this benefit. C. A Health incentive account will be available for employees enrolled into one of the medical plans offered by the City. A Health Savings Account (HSA) will be available to qualified employees enrolled into the High Deductible Health Plan. An HRA will be available to employees not enrolled in a qualifying High Deductible Health Plan. D. The City of Sarasota shall contract for annual physicals, cardiac stress exams, and hearing tests for members of the Bargaining Unit. In addition, members of the
COMPREHENSIVE HEALTH CARE PROGRAM. Section 1. The Union will provide and administer a comprehensive group health and dental plan for all members of the Bargaining Unit. Section 2. The City will continue to pay eight hundred thirty four dollars ($834) per month to the plan for each eligible employee. Effective January 1, 2018, the City will begin making contributions for all sworn budgeted positions, including vacancies. Effective January 1, 2019 and January 1, 2020, the City contribution shall be adjusted, if necessary, to equal any higher contribution paid by the City for any other City bargaining unit. Any amounts above the City’s contribution will be paid by the employee. Section 3. The Union agrees that the City’s payments of the sums set forth in subparagraph Section 4. The Union will provide the City’s Finance Director, with a written profit and loss report for each quarter showing the Union’s hospitalization insurance plan’s income, expenses and claims paid. Section 5. The parties understand that it is the intent of FOP that the Union health plan is not a City of Fort Lauderdale benefit plan therefore FOP shall not use “City of Fort Lauderdale” in its plan documents. Section 6. The City agrees to reimburse the Union health plan for the Patient Protection and Affordable Care Act (PPACA) Patient Centered Outcomes Research Institute Fee in the amount of one dollar ($1) per plan participant for Fiscal Year 2014 and two dollars ($2) per plan participant for Fiscal Year 2015. The City also agrees to reimburse the Union for the PPACA Transitional Reinsurance Fee in the amount not to exceed sixty three dollars ($63) per plan participant. Prior to any reimbursement by the City, the Union agrees that it shall submit invoices to the City evidencing the specific amounts its health plan paid for the fees and the number of participants in the Union health plan.
COMPREHENSIVE HEALTH CARE PROGRAM. ‌ Section 1. Effective January 1, 2018 or the first month following ratification of this Agreement, whichever comes later, the City agrees to make comprehensive health care available to employees and eligible dependents and contribute eight hundred dollars ($800) per month toward monthly premium costs for each eligible employee in the Bargaining Unit who elects to participate in the City-sponsored medical plan and for each vacant position in the Bargaining Unit. Section 2. The City agrees to continue substantially, if available, the present health care and dental benefits but reserves the right to change current companies, plan designs, benefits and/or offer plan alternatives to mitigate premium cost increases if the premium cost for the benefits provided exceeds the combined amount of contributions from the City and employees. Section 3. In addition to the Internal Revenue Code Section 125 pre-tax benefit for health insurance premiums, the City at its sole discretion may offer additional Section 125 benefits to bargaining unit members. The offer of additional benefits is conditioned upon compliance with Section 125 regulations including, but not limited to, the requirement that any such benefits must be made available to all eligible City employees. The City retains the sole and exclusive right to administer such benefit plan including, but not limited to, the right to develop and revise benefits and procedures.
COMPREHENSIVE HEALTH CARE PROGRAM. 1. Effective upon ratification with rates implemented as of January 2017, The City and Teamsters Local #173 agree to two medical/dental plans. 2. Employees’ single medical coverage for the Consumer Driven Health Plan will be a $0.00 premium. Employees’ single medical coverage for the Open Access Plus plan will be rates listed below. 3. Plus One and Family rates for calendar year 2017 will apply as listed in the chart below. 4. Dental will be a separate election from medical coverage. Rates are listed below. 5.   • Extended Health Care Plan ‌ The Employer shall pay the monthly premium for regular employees entitled to coverage under a mutually acceptable extended health care plan.

  • Comprehensive general liability and property damage insurance, insuring against all liability of the Contractor related to this Agreement, with a minimum combined single limit of One Million Dollars ($1,000,000.00) per occurrence, One Million Dollars ($1,000,000) Personal & Advertising Injury, Two Million Dollars ($2,000,000) Products/Completed Operations Aggregate, and Two Million Dollars ($2,000,000) general aggregate;

  • HEALTH PROGRAM 3701 Health examinations required by the Employer shall be provided by the Employer and shall be at the expense of the Employer. 3702 Time off without loss of regular pay shall be allowed at a time determined by the Employer for such medical examinations and laboratory tests, provided that these are performed on the Employer’s premises, or at a facility designated by the Employer. 3703 With the approval of the Employer, a nurse may choose to be examined by a physician of her/his own choice, at her/his own expense, as long as the Employer receives a statement as to the fitness of the nurse from the physician. 3704 Time off for medical and dental examinations and/or treatments may be granted and such time off, including necessary travel time, shall be chargeable against accumulated income protection benefits.

  • Comprehensive Evaluation The Comprehensive evaluation is a growth-oriented, teacher/evaluator collaborative process that requires teachers to be evaluated on the eight (8) state criteria. A teacher must complete a Comprehensive evaluation once every six (6) years. Subsequent years they will be evaluated on a Focused evaluation, unless they have received a Basic or Unsatisfactory rating on their final comprehensive summative evaluation. Then they shall continue using the Comprehensive evaluation for the following year. All teachers during their provisional status must be on the Comprehensive evaluation.

  • Health and Safety Plan Consultant shall prepare and submit a Health and Safety Plan (“HASP”) for the portion of Consultant’s work that will involve field work, assessments, or investigations of certain Project elements. The HASP shall describe how Consultant plans to complete field work, assessments, and/or investigations at the RWF. Consultant’s HASP must comply with the CIP HASP and shall be updated as new conditions are encountered.
