CONFIRMATION OF CANCELLATION. You must confirm the cancellation with the Cardholder in writing. The confirmation must include the following: (a) Cardholder’s name, account number, and the expiry date embossed on the Credit Card; (b) the cancellation code; and (c) any other relevant cancellation details.
