The Continuum of Care Program clause establishes the framework for providing a coordinated range of services and support to individuals experiencing homelessness. Continuum of Care Program. Contractor shall provide enhanced rehabilitation programs for offenders/residents effective July 1, 2019 as set forth in Exhibits E, H, I and K1 and K2 of Amendment No. 6 are incorporated by reference, and Exhibit D, J – FY 19/20 and Exhibit L – FY 19/20 are attached hereto. The enhanced rehabilitation program shall include additional program staff, revised/expanded program curriculum integrated with select post release assistance in the areas of housing, employment, and treatment. Contractor shall coordinate with DMS and the Florida Department of Corrections (FDC) to access offender recidivism results at one (1) year, two (2) year, and three (3) year intervals following offender release.

  • Dependent Care Assistance Program The County offers the option of enrolling in a Dependent Care Assistance Program (DCAP) designed to qualify for tax savings under Section 129 of the Internal Revenue Code, but such savings are not guaranteed. The program allows employees to set aside up to five thousand dollars ($5,000) of annual salary (before taxes) per calendar year to pay for eligible dependent care (child and elder care) expenses. Any unused balance is forfeited and cannot be recovered by the employee.

  • Office Visits (other than Preventive Care Services) This plan covers office and clinic visits to diagnose or treat a sickness or injury. Office visit copayments differ depending on the type of provider you see. This plan covers physician visits in your home if you have an injury or illness that: • confines you to your home; or • requires special transportation; and • because of this injury or illness, you are physically unable to travel to the provider’s

  • Educational Program A. DSST PUBLIC SCHOOLS shall implement and maintain the following characteristics of its educational program in addition to those identified in the Network Contract at DSST ▇▇▇▇ MIDDLE SCHOOL (“the School” within Exhibit A-3). These characteristics are subject to modification with the District’s written approval:

  • Pharmacy Services The Contractor agrees to comply with the requirements regarding covered pharmacy and over-the- counter (OTC) benefits. The Contractor will comply with the EOHHS Pharmacy Home Program and the Generics First Initiative, including the maintenance of the drug formulary in accordance with the direction of the EOHHS Pharmacy Committee.

  • Medical Services Plan 10.1.1 Regular Full-Time and Temporary Full-Time Employees shall be entitled to be covered under the Medical Services Plan commencing the first day of the calendar month following the date of employment. 10.1.2 The City shall pay one hundred percent (100%) of the premiums required by the Plan.
