Coverage Changes. When Allstate broadens coverage during the premium period without charge, you have the new features if you have the coverage to which they apply. Otherwise, the policy can be changed only by endorsement. The coverage provided and the premium for the policy is based on information you have given us. You agree to cooperate with us in determining if this information is correct and complete. You agree that if this information changes, is incorrect or incomplete, we may adjust your coverage and premium accordingly during the policy period. Any calculation of your premium or changes in your coverage will be made using the rules, rates and forms on file, if required, for our use in your state. The rates in effect at the beginning of your current premium period will be used to calculate any change in your premium.

Coverage Changes. When we broaden coverage during the policy period without charge, the policy will automatically provide the broadened coverage when effective in your state. We may make other changes or replace this policy, to conform to coverage currently in use at the next policy period.

Coverage Changes. If your insurance changes, please notify us before your next visit. If your insurance company does not pay your claim in 30 days, the balance will automatically be billed to you. SELF-PAYMENT Periodically, fees will be increased; no more than once per year with the courtesy of advanced notice where you are encouraged to express any financial concerns with your therapist.

Coverage Changes. Once a year, during the period of June 1 through June 25, the County shall allow bargaining unit personnel to elect to change health/vision and dental provider and category of coverage.
