Common use of CPT Clause in Contracts

CPT. CAC will provide or make a referral for Mental Health Services, Forensic Interviews, Specialized Interviews, Medical Assessments, Referrals to Community Services. CAC will provide the FNCAC with a list of all monthly services performed using FNCAC funding. Due in on the 25th of the month following the month of service. CAC to provide services and submit a Monthly Data Report to the FNCAC in accordance with the Scope of Work. Each CAC will sign their monthly CPT deliverable and the deliverable will be included with the CAC's invoice in the monthly invoice packet. If the PROVIDER fails to achieve the minimum monthly deliverable in accordance with the agreement, the PROVIDER will be assessed a financial consequence in the amount of 3% of the total monthly reimbursement request.
