Common use of CPT Clause in Contracts

CPT. CAC will provide FNCAC with a budget detailing spending of the CPT funding by July 5 2021. CAC will send CPT Budget to FNCAC by July 5 2021 in accordance with the Scope of Work. Approved budgets will be forwarded to the OAG. Each CAC will sign their monthly CPT de iverable and the deliverable will be included with the CAC's invoice in the monthly invoice packet. If the PROVIDER fails to achieve the minimum monthly deliverable in accordance with the agreement, the PROVIDER will be assessed a financial consequence in the amount of 3% of the total monthly reimbursement request.
