Common use of Credentialing Services Clause in Contracts

Credentialing Services. 15 1. ADMINISTRATOR may require, and INTERMEDIARY has agreed, if requested by 16 ADMINISTRATOR, to provide at an additional cost to COUNTY, verification of credentials or 17 “credentialing” of All Providers of Medical Services for the MSI Program. 18 2. Credentialing of all current and new Providers, if required, shall be done at a frequency 19 required by Department, in accordance with the STCs. ADMINISTRATOR anticipates credentialing 20 shall be completed at least once during the term of the Agreement for all current and new Providers 21 determined to not already be credentialed by another entity as approved by ADMINISTRATOR. 22 3. Credentials to be verified shall be as required by the STCs and/or Department, and may 23 include, but not necessarily be limited to: 24 a. State licensure 25 b. Education, training, and board certifications 26 c. DEA/CDS certification 27 d. National Practitioner Data Bank 28 e. OIG sanctions 29 f. Malpractice claims history
