Credit Monitoring. All Settlement Class Members are eligible to make a claim for two (2) years of IDX one-bureau Credit Monitoring Services, regardless of whether the Settlement Class Member submits a claim for reimbursement of documented ordinary losses, including lost time, reimbursement for extraordinary losses, or the Pro Rata Alternative Cash Payment. The Settlement Administrator shall send an activation code to each valid Credit Monitoring Services claimant within 45 days of the Effective Date that can be used to activate Credit Monitoring Services. Credit Monitoring. CareFirst BlueChoice is offering credit monitoring to the Subscriber and eligible Dependents at no additional charge through services administered by Experian. Credit monitoring is available on an opt-in basis for all eligible Members during the effective Benefit Period of their CareFirst BlueChoice health insurance policy. Eligible Members may enroll by calling the number on the back of their ID card or visiting ▇▇▇.▇▇▇▇▇▇▇▇▇.▇▇▇.
Credit Monitoring. In addition to electing a Cash Payment, Settlement Class Members may elect up to three years of three-bureau Credit Monitoring that will provide the following benefits: three-bureau credit monitoring, dark web monitoring, identity theft insurance coverage for up to $1,000,000.00, and fully managed identity recovery services.
Credit Monitoring. 24 months of 3 bureau credit and identity theft monitoring will be provided for those Settlement Class Members who elect and submit valid claims for such credit and identity theft monitoring. The credit and identity theft monitoring will have the following features:
i. Real time monitoring of the credit file at all three major credit bureaus;
ii. Identity theft insurance (no deductible) of $1,000,000; and
iii. Access to fraud resolution agents to help resolve identity thefts.

These terms are capitalized and defined in Section A below, and/or in the Individual Enrollment Agreement to which this document is attached. These procedures replace all prior procedures issued by CareFirst, which afford CareFirst Members recourse pertaining to denials and reductions of claims for benefits by CareFirst. These procedures only apply to claims for benefits. Notification required by these procedures will only be sent when a Member requests a benefit or files a claim in accordance with CareFirst procedures. SAMPLE An authorized representative may act on behalf of the Member in pursuing a benefit claim or appeal of an Adverse Benefit Determination. CareFirst may require reasonable proof to determine whether an individual has been properly authorized to act on behalf of a Member. In the case of a claim involving ▇▇▇▇▇▇/Emergent Care, a Health Care Provider with knowledge of a Member's medical condition is permitted to act as the authorized representative.
Credit Monitoring. In the event that either party is required by law to notify Individuals whose PHI was inappropriately accessed, used, or disclosed by Provider Partner, its employees, subcontractor(s) or its agents, and the PHI contains: (i) the individual’s first initial or first name, last name, and social security number; (ii) the individual’s first initial or first name, last name, and driver’s license or state identification card; (iii) the individual’s first initial or first name, last name, account number, credit or debit card number, in combination with any required security code, access code, or password that would permit access to an individual's financial account; and/or (iv) the individual’s first initial or first name, last name, and PHI, then Provider Partner and Covered Entity shall work together to structure a credit monitoring offering commensurate to the risk posed by the breach and Provider Partner shall, in any event, pay the costs of credit monitoring for one (1) year for such individuals and the costs and fees related to timely notification in accordance with law.
Credit Monitoring. Settlement Class Members are eligible for enrollment in 25 three years of Credit Monitoring Services, to include, at a minimum three 26 bureau credit monitoring. Any Settlement Class Member may elect to submit 1 a claim for three years of Credit Monitoring Services. Settlement Class 2 Members shall be entitled to seek Credit Monitoring as provided in this 3 section in addition to either Ordinary and/or Extraordinary losses set forth in 4 ¶¶ 4.2.1 and 4.2.2 or the Alternative Cash Payment set forth in ¶ 4.2.4.
