Determination of Need. DON) means the tool used by the Department or the Department's authorized representative to determine eligibility (level of care) for NF services and HCBS Waivers for individuals with disabilities, HIV/AIDS, brain injury, supportive living, and the elderly. This assessment includes scoring for a mini–mental state examination, functional impairment levels, and unmet needs for care in fifteen (15) areas including ADL and IADL. The final score is calculated by adding the scores of the mini-mental state examination, the level of impairment, and the unmet need for care. To be eligible for NF services or HCBS Waivers, an individual must receive at least fifteen (15) points on the functional-impairment section and a minimum total score of twenty-nine (29) points.
