Contractor shall comply with the requirements pertaining to the Disclosure of Ownership and Control Interest Statement contained in Exhibit G, “Program Integrity Requirements,” and submit to County the “Disclosure of Ownership and Control Interest Statement,” included herein as Exhibit H.
