Display of Notice. Pursuant to the notice published by the Department of Housing and Urban Development (“HUD”) on July 30, 2004, Agency will prominently display at each intake desk (or comparable location) the Privacy Policy provided by DCA, that explains generally the reasons for collecting Identifying Information in the HMIS and the Client rights associated with providing Agency staff with Identifying Information. It is Agency’s responsibility to ensure that each Client understands his or her rights. Additionally, if Agency maintains a public webpage, the current version of the Privacy Policy must be posted on the webpage. The current form of Privacy Policy, which may be modified from time to time at DCA’s discretion, is attached to and incorporated into this Agreement by reference, and is available from DCA or on its website, ▇▇▇▇://▇▇▇.▇▇▇.▇▇.▇▇▇/housing/specialneeds/programs/hmis.asp
