Common use of Doctor’s Notes Clause in Contracts

Doctor’s Notes. a. A certificate of illness may be required at any time from a physician; this requirement is mandatory for absences of three (3) or more consecutive days. b. Where a pattern of misuse can be established of three (3) separate short term occurrences, the Employer may request verification of disabling illness for the fourth and succeeding illness in the sick leave year. Employer to cover the cost of doctor’s note if required under the terms described in this Article.
