Common use of Document Disclosures Clause in Contracts

Document Disclosures. Document such disclosures of PHI and information related to such disclosures as would be required for Covered Entity to respond to a request by an Individual for an accounting of disclosures of PHI in accordance with 45 CFR § 164.528. Within ten (10) business days of a written request by Covered Entity, Physicians Insurance agrees to provide to Covered Entity information collected in accordance with this Section in order for Covered Entity to respond to a request by an Individual for an accounting of disclosures of PHI in accordance with 45 CFR § 164.528. If Physicians Insurance receives a request from an individual or an individual’s designee for PHI, Physicians Insurance shall forward any such request to Covered Entity within ten (10) business days and will coordinate any responsive communication to the request as directed by Covered Entity.
