Employee to Inform. (a) The employee shall make every reasonable effort to advise the supervisor or designated person in charge a minimum of two hours prior to an 0700 scheduled shift and a minimum of three hours prior to all other scheduled shifts of her/his inability to report to work because of illness or injury, the nature of the illness or injury, and the probable date of her/his return to work. (b) Employees who are absent from work because of sickness shall contact their supervisor or the designated person in charge on a regular basis regarding the status of their condition and/or the anticipated date of return to work. (c) Employees who have been absent from work due to illness or injury must provide sufficient notice to the Employer prior to their return to work. (d) The Director of Care or Director of Community Care or designate may require a Certificate of Fitness (Appendix D) prior to the employee resuming work. In such cases, the Director of Care or Director of Community Care or designate shall inform the employee prior to their expected date of return to provide enough time for the employee to obtain the required certificate.
