Common use of Enrollee Notification of Terminated Primary Care Provider Clause in Contracts

Enrollee Notification of Terminated Primary Care Provider. The MCO, or if applicable its subcontractor, shall make a good faith effort to provide written notice of the termination of a Participating Provider, within fifteen (15) days after the MCO’s, or if applicable its subcontractor’s, receipt or issuance of the Participating Provider termination notice, to an Enrollee who receives his or her Primary Care from, or was seen on a regular basis by, that Participating Provider. A sample Enrollee notice must be prior approved by the STATE. The MCO must provide the following information to the STATE: (A) Date the contracted Provider will no longer be available to Enrollees; (B) Number of Enrollees affected in each Minnesota Health Care Program; (C) Impact on the MCO’s Provider network; and (D) Remedy offered by MCO to alleviate the situation
