Evaluation Schedule. 1. All District teachers shall complete the Professional Growth Plan Form by October 1st regardless of observation/evaluation year (Appendix D-2). 2. A teacher rated Accomplished on his/her last summative evaluation shall be formally evaluated every three (3) years, as long as the Student Growth Measure remains at expected or above. A teacher rated Skilled on his/her last summative evaluation shall be formally evaluated every two (2) years, as long as the Student Growth Measure remains at expected or above. 3. During a full observation/evaluation year, teachers shall be formally evaluated through two (2) formal observations, as well as informal observations and/or walkthroughs.
