Experience Required. As necessary to provide the services detailed in Scope of Services/Deliverables Letters of support from local school boards across the agency’s region Letters of support from local school principals Letters of support from local DCBS staff who will be involved in the program Children served will reside with the caregiver on a regular basis Referrals for this program will only be accepted by the school districts selected Children served will be from ages 5-11 who are truant or at-risk of becoming truant How many families do they intend to serve? Administrative fees Salaries including fringe Line item budget Budget narrative Referral criteria Referral process School policy concerning truancy School collaboration including the designated personnel assigned to the program DCBS collaboration including the designated personnel assigned to the program Role of CCC designated CCC staff Process of interventions Description of the family engagement meetings to include: participants, confidentiality, consistent meeting goals, rules for participation, action plan development (including resource linkages), and follow-up process. Notification process to families regarding referral Please describe meeting: How will it be strengths based? How will parents feel empowered? How will needs be assessed? How will parents know specifics about the school policies? How will a plan of action be developed? How will DCBS be notified of the family’s plan of action? How will referrals to community partners for risk factors identified be made following the meeting? What forms will be developed/utilized? (One of the forms must include the following language: “Failure to comply with this plan may result in a referral to CPS.”) What signatures will be required? What monitoring will be completed once the meeting is adjourned by the CCC vendor? How will future unexcused absences/tardiness be handled? How will DCBS be notified of the family’s noncompliance?
