Form of Proposal. Each Bidder shall submit the Bid in four parts (original Bid Form, marked original, with attachments and three (3) copies of the Bid Form and all attachments) and indicate his base Bid price and any alternative(s) that may be included in the proper space(s). The base Bid is the sum of all pay item totals and the County reserves the right to correct errors in pay item totals arising from incorrect extensions. See "Bid Errors", Item 5.
Form of Proposal. All bids must be made on the attached form of proposal. Bid blanks must be completed and clearly filled in and must be free from alteration either by erasure or interlineations, or otherwise the bid proposal will be voided. Bids must be properly signed in ink by the bidder or by an authorized official or agent when the bidder is a firm or corporation. When the bid is made by a firm, the signature must include the firm name, and the signature of member thereof. When made by a corporation, the signature must contain the name of the corporation followed by the signature of the official or person authorized to bind it in the matter and with proof of his authority. When filed, the bid with the accompanying bid security must be enclosed together in a sealed envelope, clearly marked on the outside with the bid number and project name, addressed to the City Clerk, Sullivan, Missouri. The bidder shall designate on the bid blank his official address to which all communications can be mailed. No facsimiles will be accepted.

Form of Proposal. Attached to this Bid is the form of proposal which shall be used by all prospective bidders to submit a response to this Bid. The form shall be completely executed prior to submittal to the issuing office. If any blanks have not been filled in, Luzerne County may immediately reject the proposal as incomplete. Form of Proposal.
21.1 The submission must contain the following:
21.1.1 One (1) document (PDF preferred) entitled “Technical Proposal” which will NOT contain any references to pricing or cost. This will include a “Cover Letter” and a “Detailed Response / Description of Services” being proposed.
21.1.1.1 The “Cover Letter” shall be signed by an official who has the legal authority to bind the company to the terms of the proposal for a minimum of 90 days. The letter shall:
21.1.1.1.1 Be on official letterhead.
21.1.1.1.2 Include the name and title of the Chief Executive Officer or other individual authorized to legally bind the company.
21.1.1.1.3 Include the identification of a primary contact and that person’s title, address, phone number, fax number and email address.
21.1.1.1.4 State that the applicant accepts the terms, conditions, criteria and requirements set forth in the RFP.
21.1.1.2 The “Detailed Response / Description of Services” shall include as a minimum, documentation to support the requirements outlined in Sections 4.0 – Scope of Work through 9.0 –

Form of Proposal a. The proposal shall provide the personnel available to complete the task CV’s stating qualifications, experience and clearances.
b. When a supplier is interested in bidding for this requirement an NDA must be signed before further information is disclosed.
c. The proposal from the supplier is to determine if they can provide evidence of:
i. A demonstrable understanding of required outcomes.
ii. Details of the personnel to be involved including their role for this project and their relevant experience.
iii. Arrangements for managing this work and quality assuring outputs, including how you would like to work with the customer during the project.
iv. A breakdown of costings, including grades and rates per team member.
d. Evaluation criteria, the basis on which the contract will be awarded including the weightings of the sections / questions 80% technical 20% cost. The criteria could include:
i. Suitability of methodology ii. Experience in the area iii. Evidence of understanding the brief iv. Whole life costs including equipment and T&S
Form of Proposal. For any proposed transfer, the Estate shall deliver, not later than thirty (30) days prior to the consummation of such Proposed Transfer, (i) a purchase agreement signed by the Estate and the proposed transferee (a “Transfer Agreement”), and (ii) documentation, reasonably acceptable to FalconStor, showing that the proposed transferee has funds available to consummate the Proposed Transfer. Such Transfer Agreement shall contain all of the terms and conditions of the Proposed Transfer including a clause referencing this right of first refusal and stating that consummation of the Proposed Transfer is contingent on FalconStor’s decision not to exercise its right of first refusal. The consideration proposed to be paid for the Estate FalconStor Shares must be cash and cannot be contingent on any future event.
