Form Protocol. The Settlement Administrator shall gather and review the Claim Forms received pursuant to the Agreement and fulfill valid claims. 1. Settlement Class Members who submit a timely and valid Claim Form shall be designated as Authorized Claimants. The Settlement Administrator shall examine the Claim Form before designating the Settlement Class Member as an Authorized Claimant to determine that the information on the Claim Form is reasonably complete and contains sufficient information to issue the Settlement payment to the Settlement Class Member. 2. No Settlement Class Member may submit more than one Claim Form. The Settlement Administrator shall identify any Claim Forms that appear to seek 3.
