Required Forms Employees who want to challenge minor discipline must file a grievance on an official grievance form or a writing that contains the name of the grievant; the grievant’s department, classification, and worksite; the effective date and nature of the disciplinary action imposed; the remedy requested; and the signature of the employee and/or employee’s representative.
