Common use of Functional Assessments Clause in Contracts

Functional Assessments. a. The Health Plan shall ensure that all Behavioral Health Care Providers administer functional assessments using the Functional Assessment Rating Scales (FARS) for all Enrollees over the age of eighteen (18) and Child Functional Assessment Rating Scale (CFARS) for all Enrollees age eighteen (18) and under. b. The Health Plan shall ensure that all Behavioral Health Care Providers administer and maintain the FARS and CFARS, according to the FARS and CFARS manuals, to all Enrollees receiving Behavioral Health Services and upon termination of providing such services. c. The results of the FARS and CFARS assessments shall be maintained in each Enrollee's Medical Record, including a chart trending the results of the functional assessments. d. The Health Plan shall submit the FARS/CFARS reports as required in Section XII, Reporting Requirements.
