Granting a Leave of Absence. A. All leaves of absence must be applied for in writing to the Human Resource Services department. Requests for medical leaves, whether paid or unpaid, must be accompanied by a healthcare provider’s verification. B. Employees who take leaves of absence under the provisions of the Family and Medical Leave Act (FMLA) of 1993 shall be entitled to all benefits provided under the Act if they are eligible and qualified for such benefits.
