Grievance Statement. Except at the informal stage, all grievances shall include: the name and position of the aggrieved party; the identity of the provision of law, the Agreement, policies, etc. involved in said grievance; the time when and the place where the alleged events or condition constituting the grievance existed; the identity of the party responsible for causing said events or conditions, if known to the aggrieved party; and a general statement of the nature of the grievance and the redress sought by the aggrieved party.
