House Calls. We cover doctor visits in your home if you have a condition due to an injury or illness which: • confines you to your home; • requires special transportation; or • requires the help of another person. In a Doctor’s Office Our allowance for an office visit includes medical supplies provided as part of the office visit. See the Summary of Medical Benefits for benefit limits and the amount that you pay for each service. When physician services are rendered in a doctor’s office, other than an office visit examination, the amount that you pay is based on the type of service being rendered. For surgical services (including but not limited to sutures, fracture care, and other surgical procedures) see Section 3.34 - Surgery Services. For diagnostic imaging, lab and machine tests see Section 3.36.

House Calls. (April 1, 2013) (a) A new fee code, at the same value as the A901, will be developed for house calls to homebound and frail elderly patients. The definition of “homebound and frail elderly patient” for the purposes of this fee code shall be developed by the PHCC. (b) The current premiums for house calls shall be revised as follows: Home Visits Necessary annual criteria 3 or more patients patients served and 6 or more patients served and 17 or more patients served and 32 or more served and 12 or more encounters 24 or more encounters 68 or more encounters 128 or more encounters Annual Bonus $1,500 $3,000 $5,000 $8,000  All family physicians are eligible for level A & B  Only PEM Physicians are eligible for level C & D (c) When a CCM, FHG or FHN physician provides more than 68 house calls per year to more than 17 patients (Level C), physicians will receive a 20% premium on the additional house call services if at least 75% of the house calls were provided to their enrolled and non-enrolled homebound and frail elderly patients during the fiscal year. This will be made as an annual payment after year end. (d) When a FHO physician provides more than 68 house calls per year to more than 17 patients (Level C), all subsequent house calls to home bound and frail elderly patients will be paid out of basket. Further, FHO physicians will also receive a 20% premium on the additional house call services if at least 75% of the house calls provided to their enrolled and non-enrolled homebound and frail elderly patients during the fiscal year. This will be made as an annual payment after year end. (e) The payment of all A901 fee codes billed by a physician for services provided to patients enrolled to a physician in a different PEM group will be reduced by 50%. The Parties agree to review the impact of this reduction 6 months following its effective date.

House Calls. House calls are at the Provider’s sole discretion and must be scheduled at least one-day in advance and are subject to Provider’s availability. There is no additional fee while Practice is waiting for its physical location to open. Once opened, home visits are $100.00 each if Patient resides within 15 miles from Practice’s location. If greater than 15 miles, each additional mile is $8.

  • Mass Calling CLEC and Sprint shall cooperate and share pre-planning information, where available, regarding cross-network call-ins expected to generate large or focused temporary increases in call volumes, to prevent or mitigate the impact of these events on the public switched network. Mass calling numbers cannot be used in conjunction with INP.
