If you want to be excluded, you must either complete submit the Opt-Out Form availablerequest for exclusion through the Settlement Website or write the Settlement Administrator stating: (a) the names and case numbers of the Action; (b) the full name and the unique identification number for the Settlement Class Member assigned by the Settlement Administrator;
