Common use of Immunization Requirements Clause in Contracts

Immunization Requirements. Provider shall use vaccines available without charge to children 18 years old or younger under the Vaccine for Children program. Such immunizations shall be given in conjunction with EPSDT/well child visits or when other appropriate opportunities occur. Provider shall report required immunization data to the Nebraska State Immunization Information System, administered by Department of Health and Human Services/Division of Public Health.
