The Incident Reporting and Management clause establishes the procedures and responsibilities for promptly reporting, documenting, and addressing incidents that occur during the course of an agreement or project. Typically, this clause requires parties to notify each other of any accidents, breaches, or unexpected events within a specified timeframe and to cooperate in investigating and resolving such incidents. Its core function is to ensure transparency, facilitate timely response, and minimize the impact of incidents by providing a clear framework for communication and resolution.
Incident Reporting and Management. NHS England Lancashire and South Cumbria has a statutory responsibility to ensure that robust systems are in place for reporting, investigating and responding to serious incidents so that lessons are learned and appropriate action taken to prevent future harm. The Nursing and Quality Directorate at NHSE will:  Maintain oversight and surveillance of serious incident management within NHS funded care and seek assurances that providers have systems in place to appropriately manage serious incidents in the care they commission. Feedback from this information will be reported through the Lancashire and South Cumbria Quality Surveillance Group, in line with statutory responsibilities and also through the local NHS England co-ordinated Lancashire and South Cumbria quality improvement groups, networks and collaborative.  Review trends and analyse quality identifying issues of concern providing the wider system with intelligence gained and lessons learned. Feedback from this information will be reported through the Lancashire and South Cumbria Quality Surveillance Group, in line with statutory responsibilities and also through the local NHS England co-ordinated Lancashire and South Cumbria quality improvement groups, networks and collaborative.
Incident Reporting and Management. If you have witnessed a player violating the SYFA Code of Conduct or have been subject to a player’s unsportsmanlike conduct, contact the player’s head coach immediately. If the head coach is unavailable or if you believe it would be inappropriate to contact the head coach, contact an SYFA Coach of Coaches. If the unsportsmanlike conduct is exhibited by a parent, coach or board member, contact an appropriate board member. Upon receiving a report of unsportsmanlike conduct, the head coach will immediately report the incident to the SYFA Coach of Coaches. The Coach of Coaches will inform an SYFA Commissioner. Depending on the severity of the incident, the Commissioner may inform the full SYFA board of directors. All parties are encouraged to raise concerns, in good faith, without fear of reprisal or retaliation. SYFA will not tolerate retaliation against anyone who reports a concern in good faith. Anyone who engages in retaliation is subject to disciplinary action.
Incident Reporting and Management. The Contractor shall inform the COR, VA PM and assigned local Information Security Officer (ISO) of any security events and the Privacy Officer (PO) for any privacy violations within one hour of occurrence. Contractor will provide updates on the reported security/privacy events until closed by the ISO/PO.

The Programme Operator shall ensure that suitable and sufficient monitoring and reporting arrangements are made with the project promoters in order to enable the Programme Operator and the NFP to meet its obligations to the donors. When reporting on progress achieved in Annual and Final Programme Reports, the Programme Operator shall disaggregate results achieved as appropriate and in accordance with instructions and templates received from the FMO.
