Individual Records. Pharmacy must maintain an individual record of administration including: individual’s name, address, telephone number, date of birth, allergies and gender; vaccine administered, the manufacturer, expiration date, lot number, site of administration, and dose administered; date of original order and the date of administration(s); name and address of Authorizing physician, and the name and address of Pharmacist administering the dose, and
Individual Records. 10 Pharmacy must maintain an individual record including each vaccine administered. These records must include a copy of the written protocol, name and address of patient, age if under fourteen
Individual Records. The Engager shall ensure that a record is kept for each child, and the record must be available to the Child Supervisor at all times. The record must include the following particulars: (i) the name, address, and home telephone number of the child; (ii) the name, address, and both home and business telephone numbers of the child’s parents or legal guardian; (iii) the name, address, and both home and business telephone numbers of any other person nominated by the child’s parents or legal guardian as being a person responsible for the child in the event of the parents being unavailable; (iv) the child’s date of birth; (v) the name, office, and emergency contact telephone number of the child’s Doctor; and (vi) any medical information which may be relevant to the Child Supervisor or the Engager in protecting the child’s health such as allergies.

Pharmacy must maintain an individual record of administration including: individual name, date of birth, gender, and contact information; Pharmacist and Authorizing Practitioner name; written informed consent or electronic copy; date of administration; 2. In the event of administration of epinephrine or diphenhydramine, the record must include Pharmacist name who administered emergency drug; date of administration; street address or location of building where emergency medication administered; and name of drug administered, dose, route of administration, expiration date, manufacturer and lot number.
Individual Records. Pharmacy must maintain an individual record of administration including: individual name, address, and date of birth; date of administration, vaccine given, site of administration, manufacturer, lot number, and expiration date of vaccine; name and address of primary health care provider (PCP) and date report was sent to PCP; name of the Vaccine Information Sheet (VIS) given; and name and signature of Pharmacist administering the vaccine.

Pharmacy must maintain records for at least seven (7) years after administration of vaccine.
Individual Records. Pharmacy must maintain an individual record of administration including: individual name, address and date of birth; individual responses to screening questions; administration date, vaccine given, site of injection, dose, manufacturer, lot number, expiration date; signed informed consent form; date VIS was provided and version of VIS provided; and signature or identifiable initials of person administering vaccine.9 Pharmacy must maintain all required records in a manner that is uniformly maintained, readily retrievable, and available for inspection for a period of three (3) years.10 Immunization records must be maintained separately from other records at Pharmacy.11
Individual Records. Pharmacy must ensure that an individual record of administration is maintained for a minimum of two (2) years including: individual name, address, and date of birth; date and site of administration; vaccine given, manufacturer, lot number, expiration date; name and address of primary health care provider as identified by patient; name or identifiable initials of person administering vaccine; documentation of informed consent; and the nature of any adverse reaction and any resulting notifications.11
