Success Measures This scheme will impact on the following BCF metrics: • Percentage of people who are discharged from acute hospital to their usual place of residence: The percentage of Hillingdon residents aged 18 and above discharged to their usual home. The target for 2021/22 is 91%. • Permanent admissions to care homes metric: Reduction in permanent admissions to care homes per 100,000 65 + population. The ceiling for 2022/23 is 776.3. • Still at home 91 days after discharge metric: An increase in the percentage of people aged 65 + still at home 91 days after discharge. The 2022/23 target is 90.5%. Other success measures include: • Daily bed occupancy rate at Hillingdon Hospital: The bed occupancy rate should be at no more than 90%. • Length of stay of seven days or more (Hillingdon Hospital): Percentage of people in hospital with a length of stay of seven days or more (known as ‘stranded patients’) should be no more than 30% of the bed base, i.e., 90 based on 315 core beds. • Out of hospital capacity: Health and social care capacity at no more than 90% utilisation.
