Key Attributes. A PMH has a number of key attributes that define how a practice can support patients, including through team-based care. Those key attributes are the following: i. Person centred, whole-person care • Care is easily navigated and centred on the needs of the individual, family and community. • Individuals are empowered in optimal self-management and contribute to the development and assessment of the practice/clinic and community care models. • Care will be delivered in a culturally appropriate manner with recognition of social determinants of health and attention to marginalized populations.

The currency of the listing should be identified with the latest publication date identified along with an indicative frequency of update. c. The listing should be available in a web based version to maximise access, and should provide users with the ability to search by education provider, geographical region or engineering discipline if more than (say) 100 programmes are listed.   • Green Attributes Seller hereby provides and conveys all Green Attributes associated with all electricity generation from the Project to Buyer as part of the Product being delivered.   • Gains Where an error or omission has occurred under this Agreement, the Custodian may take such remedial action as it considers appropriate under the circumstances and, provided that the Fund is put in the same or equivalent position as it would have been in if the error or omission had not occurred, any favorable consequences of the Custodian’s remedial action shall be solely for the account of the Custodian.

  • Imputed Income The Bank shall impute the economic benefit to the Executive on an annual basis, by adding the economic benefit to the Executive’s W-2, or if applicable, Form 1099.
