Limitations on Abortions. For abortion services provided under the Agreement, Provider shall attach a certification statement to the claim form certifying in writing that, on the basis of Provider’s or Contracted Provider’s professional judgment, the life of pregnant Covered Person would be endangered if the fetus were carried to term. The certification statement must contain the diagnosis or medical condition that makes the pregnancy life endangering.
