Common use of Local Service Request Clause in Contracts

Local Service Request. Orders for resale of services will be placed utilizing standard Local Service Request ("LSR") forms. GTE will continue to participate in industry forums for developing service order/disconnect order formats and will incorporate appropriate industry standards. Complete and accurate LSR forms (containing the requisite end user information as described in the Guide) must be provided by NEW PHONE before a request can be processed. 3.1.1 GTE will accept orders for As-Is Transfer of services from GTE to NEW PHONE where GTE is the end user’s current local exchange company. GTE cannot provide an As-Is Transfer of service from another CLEC selling GTE’s services to NEW PHONE.
