Membership Card. An integrated Medicare and Medicaid membership card that conforms to the requirements in Minnesota Statues, § 62J.60 subd. 3, and has been approved by the STATE prior to printing, that identifies the Recipient as an MCO Enrollee and contains an MCO telephone number to call regarding coverage, procedures and Grievances and Appeals. The membership card shall demonstrate that the Enrollee is a Recipient of Minnesota Health Care Programs, either by printing the Enrollee’s STATE PMI number on the card, or by other reasonable means. The card may include data elements required by CMS for Medicare eligible Enrollees.
Membership Card. 4.1 Membership cards are issued to all Members and must be shown at the Club to gain entry on each visit made to the Club or its golf course(s).
4.2 You must inform us as soon as possible if your membership card is lost or stolen. We reserve the right to charge you £20 for replacing a lost, stolen, or damaged membership card.
4.3 Membership cards are personal to you. You must not share, sell, lend, or otherwise allow any other person to use your membership card. Any fraudulent use of the membership card by you or any third party to whom you give (or allow to use) your membership card may result in termination of the Contract by us and cancellation of your Membership.
4.4 Where the Contract is terminated by us in accordance with clause 4.3, no refund for any pre-paid Membership Fees (or any other pre-paid fees or charges) will be payable by us. For those Members who pay on a monthly basis, where clause
4.3 applies, we shall be entitled to issue you with an invoice for the Membership Fees for the remainder of the applicable Membership Year, with payment by you due on demand.
4.5 The membership card remains our property and must be promptly returned to us, upon termination or expiry of your Membership for any reason.
Membership Card. 4.1 A membership card will be issued to each Member when they join a centre.
4.2 Your membership card is non-transferable. You must not allow anyone else to use your card or membership number. Cards being used by any other person other than the authorised member will result in that membership being cancelled.
4.3 If a membership card is lost, stolen or damaged the management will issue a replacement card for a nominal fee of £2.00.
4.4 A valid membership card must be presented clearly to the receptionist or validated by the computer system at the kiosk each time you use the facility, before proceeding into the centre. Any Member attending a centre without a valid membership card will be asked for proof of identification. After the 3rd consecutive visit without a membership card, the member will be asked to pay the fee for the activity they are attending.

Membership Card. The Employer shall include a Union Membership Card in each employee’s employment paperwork. Membership Card. A Membership Card will be issued to each Member when they join the gym and registered on the finger recognizing technology. There is a fee of R150.00 for replacement of lost, stolen, or damaged cards. Each Member must have his/her card swiped for security and verification on each visit to the gym before proceeding into the gym. Any Member attending a gym without a valid Membership Card may be asked for proof of identification. A Member must not lend, transfer or assign his Membership Card to anyone else as membership is personal and covers only the Member’s use of the gym. A member who allows a membership card to be used by a non-member may be expelled summarily from the club and his membership terminated. On termination of membership, for any reason, the Member is required to surrender his Membership Card to our reception.
Membership Card. 1. After the Member has paid the Membership Fee (or its proportional amount), the Member will be issued with a Membership card. The Membership card is a proof of Membership and entitles the Member to use the Services.
2. The card is personal and non‐transferable. It is forbidden to lend it to other persons. The Member must report the loss or theft of the Membership card to the Company immediately, at a reception of any Club. The Member must pay an administrative fee for a new Membership card in the amount specified in the Company’s price list valid to date.
3. The Member (who is already issued with a card) must identify himself with the Membership card upon entry of the Club. In the event of doubts regarding the Member’s identity, the Member is obliged to identify himself/herself upon request of the Company’s staff with a personal identity card, passport or a different official document bearing his photograph. If the Member fails to identify himself/herself with a Membership card or by a different means upon the Company’s staff’s request, the Member does not have the right to use the Services and the Company may deny him/her the entry to the Club.
