I understand that SOUTHWEST TECH uses weekly electronic billing (e-bill) as its official billing method, and therefore I am responsible for viewing and paying my student account e-bill by the first day of the term. Self-Payment: I understand and agree that SOUTHWEST TECH requires that I pay in full the amount of any fees, tuition, and other miscellaneous charges incurred in connection with my enrollment. I understand I may be subject to late charges and collection costs if I fail to comply in a timely manner with the payment obligations of this Agreement. • $25 enrollment fee per semester (ACH & credit card) • Nonrefundable returned payment fee if a payment is returned. • If I am terminated from the Payment Plan, I understand the tuition/fees are due immediately and I may be subject to late charges and collection costs. If a payment made to my student account is returned by the bank for any reason (check or ACH), I agree to repay the original amount of the payment plus a $35 returned payment fee to SOUTHWEST TECH. I also understand that I may be subject to late charges and collections costs if I fail to comply in a timely manner with the payment obligations of this Agreement.

I understand, if I decide to completely withdraw from ▇▇▇▇▇▇▇▇ University or initiate a course change, I will follow the instructions at ▇▇▇▇▇▇▇▇ University’s Tuition Refund Policy which I understand and agree are incorporated herein by reference. I understand Tuition and Fee Refunds associated with a withdrawal or course change are processed in accordance with the ▇▇▇▇▇▇▇▇ University Tuition Refund Policy. I understand it is my responsibility to understand the financial impact of a course change and/or withdraw from ▇▇▇▇▇▇▇▇ University prior to requesting a drop or withdrawing from the University. I understand that ▇▇▇▇▇▇▇▇ University is bound by the Family Educational Rights and Privacy Act (FERPA) which prohibits ▇▇▇▇▇▇▇▇ University from releasing any information from my education record without my written permission. Therefore, I understand that if I want ▇▇▇▇▇▇▇▇ University to share information from my education record with someone else, I must provide written permission by completing and submitting the "Request to Release Personally Identifiable and Confidential Information" form to the Office of Financial Aid and/or the ▇▇▇▇▇▇▇▇ University Business Office. I further understand that I may be subject revoke my permission at any time as instructed in the same procedure. I agree to late charges and collections costs if provide my Social Security number (SSN) or taxpayer identification number (TIN) to ▇▇▇▇▇▇▇▇ University upon request as required by Internal Revenue Service (IRS) regulations for Form 1098-T reporting purposes. If I fail to comply in provide my SSN or TIN to ▇▇▇▇▇▇▇▇ University, I agree to pay any and all IRS fines assessed as a timely manner with result of my missing SSN/TIN. I consent to receive my annual IRS Form 1098-T, Tuition Statement, electronically from ▇▇▇▇▇▇▇▇ University. I understand that if I do not consent to receive my Form 1098-T electronically, a paper copy will be provided. I understand that I can withdraw this consent or request a paper copy by contacting the payment obligations of this Agreement▇▇▇▇▇▇▇▇ University Business Office.

I understand that if I do not consent to receive my Form 1098-T electronically, a paper copy will be provided. I understand that I can withdraw this consent or request a paper copy by following contacting the business office. Student Age I understand and agree that SOUTHWEST TECH requires that if I pay in full am younger than the amount applicable age of any feesmajority when I execute this agreement, tuitionthe educational services provided by Franklin College are a necessity, and other miscellaneous charges incurred in connection with my enrollment. Fees are due I am contractually obligated pursuant to the first day “doctrine of necessaries.” Governing Law I understand and agree that this agreement shall be governed by, and construed under, the internal laws of the termState of Indiana. I understand I Entire Agreement This agreement supersedes all prior understandings, representations, negotiations and correspondence between the student and Franklin College, constitutes the entire agreement between the parties with respect to the matters described, and shall not be modified or affected by any course of dealing or course of performance. This agreement may be subject modified by Franklin College if the modification is signed by me. Any modification is specifically limited to late charges and collection costs if I fail to comply those policies and/or terms addressed in the modification. I further understand that failure to review my e-bill account does not constitute a valid reason for not paying my bill on time. If I am an online student residing outside of the state of California, I may be covered by a cancellation and refund policy as required by the higher education regulatory agency in my state that will take precedence over the polices listed here. If I decide to completely withdraw from Claremont Graduate University, I will follow the instructions at ▇▇▇▇▇://▇▇.▇▇▇.▇▇▇/registrar/students/student-Payment: status/ > Withdrawal. If I fail to register for either fall or spring semester courses, I understand and agree that SOUTHWEST TECH requires that I pay in full the amount may be involuntarily withdrawn for lack of any fees, tuitionenrollment, and other miscellaneous charges incurred that the last date of attendance and date of withdrawal will be determined by the University Registrar. I also understand that financial aid revisions may result based on federal regulations. I acknowledge that students enrolled in connection an online course or degree program are subject to the same registration deadlines and withdrawal policies that students in on-ground courses or degree programs must abide by. I further acknowledge that a withdrawal from the university or a leave of absence does not exempt me from paying tuition and fees from prior semesters and I agree to pay such fees in accordance with Claremont Graduate University payment deadlines. I agree to provide my enrollmentSocial Security number (SSN) or taxpayer identification number (TIN) to Claremont Graduate University upon request as required by Internal Revenue Service (IRS) regulations for Form 1098-T reporting purposes. Payment Plan, I understand the tuition/fees are due immediately and I may be made by credit or debit card is subject to late charges and collection costsan additional fee. A non-refundable fee is due at the time you enroll in the payment plan, the amount of the non-refundable fee will be listed in the approved miscellaneous fees approved by the Board of Trustees. If a payment made is late a late fee will be applied to the account, the amount of the late fee will be listed in the miscellaneous fees approved by the Board of Trustees.
